To submit a proposal for 2025 please fill out form below and attach the following information. Grant proposals are due by February 1, 2025. For questions or assistance please email pegfagan@aol.com.

A brief history of your organization
A brief description of the proposed project
A complete project budget
A list of other organizations asked to support this project
A list of the governing body of your organization
A copy of the Form 990 for the prior year
Your organization’s current, year-to-date fiscal statement
Your organization’s 501(c)(3) tax exemption letter
